ALL FORMS MUST BE SUBMITTED BEFORE 3:00PM ON A DAY THE OFFICE IS OPEN.  Failure to submit the form prior to then may result in the form not being processed until after the court session the next business day.
In other words, if you have a hearing scheduled for today, it may be too late to have the form processed prior to the hearing.
If you have a hearing scheduled tomorrow morning or the next business day's morning, and you submit the form at 5:00pm, it may be too late to have the form processed prior to the hearing.

A defendant that is not represented by an attorney or public defender may file a motion on their case pro se.  The definition of Pro Se is without representation.

-If you fail to complete the form completely or accurately, the form will NOT be processed.

You must enter the CASE NUMBER correctly.  Your submission will be rejected if you fail to enter the case number correctly.  

You will only enter the information to the RIGHT of 84H01

Case Number Format is 84H01-####-CM-###### for Misdemeanor cases

You must enter a number where # is shown above.  If you forgot to enter all of the numbers, the case will be rejected.

Case Number Format is 84H01-####-IF-###### for Infraction cases.

Again, it will be rejected if the case number is not correct.

Want to plead not guilty on your non-court required case?  Use the ENTRY OF DENIAL form
If you are currently scheduled to appear in court, you cannot use this form.

Have a financial situation that prohibits you from paying your court costs and fines?  Use the PETITION TO VACATE FINES/COSTS form

You cannot use this form unless you meet one of the following situations:
-You have already appeared in court and have been assessed court costs/fines
-You have a default judgment entered against your case
-You have a case referred for Failure To Pay
