This letter explains the process for becoming an an approved hauler to deliver wastewater from residential septic tanks and commercial grease traps to the Terre Haute wastewater treatment facility. It includes information for state and local licenses, billing rates, delivery locations, hours of operation, and safety and conduct rules.

This form is to be completed upon delivery of each waste load at the Terre Haute treatment facility. All waste loads are subject to verification; therefore, upon request EACH COMPONENT of your delivery must be verifiable by a copy of a dated customer receipt showing the origin, nature and volume of waste removed by your hauling company.
